Position summary
The LSW provides counseling and supportive services to individuals, families, and groups. The role includes clinical assessment, counseling, treatment planning, documentation, coordination of care, crisis intervention, referrals to community resources, and collaboration with other providers.
The LSW works under appropriate clinical supervision and is expected to provide person-centered, ethical, confidential behavioral health services.
Responsibilities
Conduct clinical and psychosocial assessments.
Develop individualized treatment and service plans.
Provide individual, group, and/or family counseling services as assigned.
Maintain accurate and confidential client records.
Monitor progress and update treatment plans as appropriate.
Participate in case reviews, supervision sessions, and staff meetings.
Collaborate with interdisciplinary team members and community providers.
Refer clients to appropriate community resources and services.
Follow HIPAA, New Jersey licensing requirements, professional ethics, and our policies.
Qualifications
Current New Jersey Licensed Social Worker (LSW) license.
Master's Degree in Social Work (MSW) from an accredited program.
Knowledge of counseling, crisis intervention, case management, and clinical documentation.
Strong communication and interpersonal skills.
Ability to work with individuals from diverse backgrounds.
Ability to complete required background checks and agency onboarding requirements.
