NJ DDD Survey Outreach Begins in October: Your Experience Can Help Inform Services
Reviewed October 8, 2026. People receiving New Jersey DDD services may hear from the state this month about sharing their experiences through the National Core Indicators In-Person Survey.
An invitation, not a requirement
DDD’s 2026–2027 guidance says outreach by phone or email begins in October. Participation is voluntary, and participants may stop at any time. If someone agrees, DDD schedules an in-person or online meeting and emails the details.
Whose experiences are included?
DDD describes the survey as involving randomly selected adults age 21 or older who receive a DDD-funded service beyond support coordination. It helps the state understand people’s experiences with services. The person answers the first section; a proxy may answer the second section if needed.
Check the invitation through the official source
If you have questions about an invitation, use DDD’s official NCI page and its listed contact, DDD.NCI@dhs.nj.gov. We encourage people to make their own participation decision and share their experiences honestly. Please do not place private survey responses or personal care information in blog comments.
